How a telehealth FCA works

Fill out the short form on this page or email us. We call to confirm an FCA is what your plan review needs, and give you the hours in writing before anything is booked.

Assessment by video
We talk through your goals and history, use standardised assessments, and watch everyday tasks in your own home on your phone or laptop. With your consent we also speak to family, support workers and treating clinicians.
- Step 3
Your report in 14 days
Findings by functional area and recommendations tied to what we observed. You read it first and we fix any factual errors before it goes to the NDIA.

Who writes your report
Every report is checked and reviewed by a senior occupational therapist.
Louise founded Peninsula Pathways OT on the Mornington Peninsula. She has 15 years of experience in NDIS assessment and writes to one standard: clear evidence, clear reasoning, nothing padded.
Your assessment is done by Louise or another experienced OT on our team, and the OT who assesses you writes and signs your report. Before it reaches you, a senior OT reviews it for clear evidence and reasoning.

When you'd book one
- A plan review where the current plan no longer fits

Common questions
Can an FCA really be done by video?
Yes. We watch you do everyday tasks in your own home rather than a clinic, and the report explains how the assessment was done so the planner can rely on it.
Is there a home visit?
No. This service is done entirely by video call, wherever you are in Australia. If something genuinely needs to be seen in person, we'll tell you before anything is booked.
How is it paid for?
From Capacity Building, Improved Daily Living, at NDIS price guide rates. We work with self-managed and plan-managed participants. We can't take NDIA-managed plans, as we're not a registered NDIS provider.
How long does it take?
Your report is back within 14 days of your final session. Tell us your plan review date when you enquire and we'll work back from it.
What do I need?
A phone, tablet or laptop with a camera, and ideally someone who can hold the phone during the home walkthrough.
Support coordinators and plan managers
Send us the referral with the plan review date. We contact your client within 2 business days, and you get a copy of the report the day it's finished.
