Privacy Policy

Peninsula Pathways Occupational Therapy (ABN 54 698 782 651) provides occupational therapy on the Mornington Peninsula and by telehealth across Australia. We handle personal and health information under the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records Act 2001 (Vic). This policy explains what we collect, why, and what you can do about it.
What we collect
Contact details: your name, phone number, email address and state. NDIS details: your plan management type, plan review date and funding information relevant to your assessment. Health information: your diagnosis, medical and social history, functional abilities, goals, assessment results and our clinical notes and reports. Information from other people: with your consent, we collect information from family members, carers, support workers, support coordinators, plan managers and treating health professionals.
How we collect it
Through our enquiry form, our referral form, email and phone, during assessment and telehealth sessions, and from the people above with your consent. Telehealth sessions are not recorded unless we ask and you agree first.
Why we use it
To respond to your enquiry, provide occupational therapy services including Functional Capacity Assessments, write your report, invoice your plan manager or you, meet our professional and legal obligations, and improve our service. We only collect health information with your consent and only what we need to provide the service.
Who we share it with
With your consent, we share your report and relevant information with the NDIA, your plan manager, your support coordinator and other people you nominate. Our assessing occupational therapists, including contractors, and the senior occupational therapist who reviews reports see your information only to deliver your service and are bound by confidentiality. We use trusted software providers for email, clinical records, telehealth, website forms and invoicing. Some of these providers may store data outside Australia; we take reasonable steps to make sure they protect it to Australian standards. We may also disclose information where the law requires or permits it, such as to prevent a serious threat to someone's life, health or safety. We never sell your information.
Cookies and advertising pixels
This website uses cookies, the Meta Pixel and Google advertising tags to measure how our advertising performs, for example to count how many people submit an enquiry after seeing an ad. We do not send the contents of our forms, or any health or NDIS information, to Meta or Google. They may use this data under their own privacy policies. You can turn off cookies in your browser settings and manage ad preferences in your Facebook, Instagram or Google account settings.
How we store and protect it
Records are kept in password-protected systems with access limited to the people who need it. We keep health records for at least 7 years after your last service, or for a person under 18, until they turn 25, as the Health Records Act 2001 (Vic) requires. After that, records are securely destroyed.
Accessing and correcting your information
You can ask for a copy of the information we hold about you, or ask us to correct it, by emailing info@peninsulapathwaysot.com.au. We respond within 30 days. If we can't give you access, we'll explain why in writing.
You can stay anonymous, up to a point
You can ask general questions without giving your name. To assess you and write a report for the NDIA, we need to know who you are.
Complaints

If you're not satisfied, you can contact the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992) or the Victorian Health Complaints Commissioner (hcc.vic.gov.au, 1300 582 113).

We may update this policy. The date at the top shows when it last changed.
