How we handle your personal and health information.
To provide occupational therapy services we collect personal information (name, contact details, date of birth, NDIS or Home Care Package details, funding arrangements) and health information (relevant medical history, functional capacity, goals, assessment results, correspondence with other providers).
To assess your needs, deliver services, write the reports your funder requires, communicate with other people in your care team with your consent, and meet our legal and professional record-keeping obligations.
With your consent, we may share information with your support coordinator, case manager, plan manager, GP, other treating practitioners, equipment suppliers or builders where relevant to your goals. We share information without consent only where the law requires or permits it, including where there is a serious and imminent risk to someone's safety.
Records are stored securely and access is limited to the practitioner. Health records for adults are retained for a minimum of seven years from the last occasion of service; for people under 18, until they turn 25. You can ask us at any time where your records are held and we will tell you.
You can request access to the personal and health information we hold about you, and ask us to correct anything inaccurate. Email info@peninsulapathwaysot.com.au and we will respond within 30 days.
Information submitted through the enquiry and referral forms on this site is delivered to us by email through our website host and stored with the enquiry.
This site uses Google Analytics to understand how people find and use it — for example, which pages are viewed and whether a visitor arrived from a search engine, an AI assistant or an ad. Google Analytics uses cookies and collects information such as your approximate location, device and browser. We see this only as combined statistics; it does not tell us who you are, and nothing you type into our forms is sent to Google Analytics. We also advertise through Google Ads, and Google may use cookies to measure whether an ad led to a visit. You can block these cookies in your browser settings, or install Google’s opt-out add-on at tools.google.com/dlpage/gaoptout.
We do not sell your information or share it with advertisers.
