We accept referrals from support coordinators, case managers, LACs, allied health colleagues, GPs, hospital discharge planners and families across the Mornington Peninsula.

If we don't have capacity, we'll say so rather than adding your client to a list they never hear about again.

What we accept.

  • NDIS participants — self-managed and plan-managed. We are not able to take NDIA-managed plans.
  • Home Care Package recipients
  • Private and self-funded clients — call to discuss

Service area is the Mornington Peninsula for home visits, plus telehealth Australia-wide where the work suits it.

Process

How to refer.

Send the referral

Email info@peninsulapathwaysot.com.au or use the form below. Include the participant's name and contact details, NDIS or HCP number, plan management type, plan dates and what you're seeking.

We respond within 24 hours

We'll confirm whether we can take the referral, what it would involve and an indicative timeframe. If we can't, we'll say so straight away.

We contact the participant directly

With your consent and theirs, we make contact and book the initial appointment at their home or by telehealth.

04

You stay in the loop

We confirm once the first appointment is booked, tell you straight away if anything is going to run late, and send you the report on completion.

Referrer questions

Do you accept NDIA-managed participants?

No. We work with self-managed and plan-managed NDIS participants and with people funded through aged care. NDIA-managed plans require an NDIS-registered provider.

How quickly can you see a new referral?

One to two weeks for a first appointment, and reports one week after the final assessment session. The capacity block at the top of this page is kept current, so it is the fastest way to check before you refer.

Do you travel to my client's suburb?

Yes — home visits across the Mornington Peninsula. Travel time affects scheduling for the southern Peninsula, so tell us the suburb and we will give you a realistic timeframe rather than an optimistic one.

Can you complete a Functional Capacity Assessment before a plan review?

Yes. Allow one to two weeks to the first appointment, the assessment itself, then one week for the report, plus time for you to read it before the review. Six to eight weeks out is comfortable.

Do you offer telehealth?

Yes, Australia-wide. Telehealth suits review sessions, coaching and some assessment work. Anything that needs a look at the physical environment — home modifications, falls risk, equipment fit — needs a home visit.

What do you need from me in a referral?

The participant's name and contact details, their NDIS or HCP number, plan management type, plan start and end dates, and what you are seeking. If there is a deadline, tell us the date up front.
